Parents Statement of Intent:
As parents, it is our intention to attend weekly Mass and practice our Catholic Christian faith. We will share in the life of the Church and create an atmosphere that will encourage our family members to love and appreciate their faith. By signing below, I certify that all information provided on this form is true and correct, and I hereby give permission for the baptism of the above named child in the Catholic Church.
Godparent Information
Baptism godparents are a sign of the whole Church’s concern for your family. Godparents are to be faith role models for your child, and as such, will promise during the baptism ceremony to help you with raising your child in the Catholic faith. A godparent is required to be Catholic and may not be the parents of the child. Godparents must be at least sixteen years of age and actively living their faith.Only one godparent is required, but two are permitted.A non-Catholic Christian who is actively living their Christian faith may be a witness to your child’s baptism along with a Catholic godparent.
